Texas Consumer Notice
Records Requests & Complaints (Effective September 1, 2025)
Under Texas Health & Safety Code §181.105, licensed behavioral health providers are required to post instructions for consumers on how to request health care records, contact the licensing authority, and file a consumer complaint.
Requesting Your Health Care Records
Clients may request copies of their health care records from Healthy Horizons Counseling, PLLC by submitting a written request.
How to request records:
Email: [your records request email]
Subject line: Records Request – [Client Full Name]
Please include the following in your request:
Your full name and date of birth
The specific records you are requesting (dates of service and type of record)
How you would like the records delivered (secure email, client portal, or mailed copy)
A signed authorization if records are being released to a third party
A phone number or email address for identity verification, if needed
Requests are processed in accordance with applicable Texas and federal law.
Any allowable fees will be discussed prior to release.

The Texas Behavioral Health Executive Council oversees licensing for professional counselors, marriage and family therapists, psychologists, and social workers.
